- When filling out
order form, please use ink for clarity.
- State quantity
and quarter desired.
- Please write your
customer number on all payments.
- If you do not
have a charge account with UGP, please include payment with your order.
- Payment for foreign
accounts must be in U.S. funds.
- There is a $10.00
charge for returned checks.
- When submitting
a change of address, include both old and new addresses.
- For returns, give
the name of the person who ordered the material and the reason for its
return. Returns must be received by us by the fifteenth of the first
month of the new quarter. You are no longer required to return complete
publications. Return only the front covers or part 1 (both copies) of
take-home papers postage paid to the following address: UGP

A detailed packing list must accompany all returns.

This reminder notice is sent quarterly three weeks before your next order is due. Please note this service replaces the order blanks that are mailed to you quarterly.
